While reading this I thought the same lol, Izumi is to cute so I think Ryouma finds it weird to be the botton.

I think it has to do more with pride than Izumi being cute. Maybe it's like he feels ashamed. (It's nothing to be ashamed about though.)
That reminds me of how my uke was (And I'm Izumi cute...androgynously getting "Minor" tickets at the movies cute xD) but I'm the seme now when he let go of his pride. And it had nothing to do with how I looked it's about admitting it and learning it doesn't make you any less manly. (Thing that yaoi manga's are unaware of)
Thanks for your insight, Fudanshi! The lack of equality in manga relationships is always a little off putting for me. I'd love to see a manga that explores getting over that sort of pride. Maybe in the future.

Love this manga, but one small thing that bugs me is when Ryouma is soo against being done by Izumi. Why would you do something to your partner that you would NEVER try? Seems pretty unfair. Yeesh.
